The BBC committed a "major breach" of industry standards by not revealing that the presenter of a film about Gaza was the offspring of a Hamas representative, according to regulatory body Ofcom.
An investigation by the media regulator into the documentary titled Gaza: How to Survive a Warzone has found that the film was "substantially inaccurate".
The head of the BBC had already issued an apology, saying there had been "a significant failing in relation to truthfulness".
The regulator has instructed the broadcaster to transmit a peak-time declaration about its decisions.
"Given this constitutes a significant violation of our code, we are directing the corporation to broadcast a statement of our conclusions against it on BBC Two at 9pm, with a date to be determined," the statement read.
'Potential to Damage Credibility'
The authority's report declared: "The inquiry we conducted concluded that the film's failure to disclose that the voice's father occupied a role in the Hamas-controlled government was materially misleading.
"This implied that the audience did not have vital facts which may have been highly relevant to their judgment of the narrator and the information he provided.
"Confidence is at the center of the relationship between a media organization and its viewers, particularly for a state-funded media outlet such as the British Broadcasting Corporation.
"This failing had the risk to undermine the very considerable degrees of credibility that the public would have placed in a BBC factual programme about the Israel-Gaza war."
The programme was pulled from iPlayer in February after the presenter's family links came to light, and in this past July an broadcaster's internal assessment found it breached the broadcaster's editorial guidelines on truthfulness.
A representative for the broadcaster said: "The regulator's decision is in line with the findings of the corporation's inquiry, that there was a serious error in the programme in relation to the broadcaster's journalistic policies on truthfulness, which reflects regulation 2.2 of the authority's broadcasting regulations.
"We have apologised for this and we comply with the authority's determination in its entirety. We will adhere to the sanction as soon as the date and wording are finalised."
